ABOUT The Joint Health & Safety Committee (JHSC)

The Joint Health and Safety Committee at McMaster Innovation Park (MIP) is steadfast in its commitment to maintaining the well-being of our stakeholders. This dedicated group leads with proactive safety measures and thorough incident review processes, ensuring all workplace safety concerns are addressed promptly and effectively. By evaluating and acting on reported incidents, the committee upholds and enhances MIP's rigorous safety standards, continuously advancing our commitment to a secure working environment.

Joint Health & Safety Committee Incident Report

At McMaster Innovation Park, safety is our highest priority. We are uncompromising in our commitment to the health and safety of our employees, subcontractors, tenants, and community. We will continually improve our processes, demonstrate leadership, and promote comprehensive safety. Despite our best efforts accidents do happen.  The Joint Health and Safety Incident Report form below is to be used by anyone affiliated with MIP (employee, tenant, or contractor), with a work-related incident.  MIP’s JHSC will review and provide actionable steps for correction based on best practices for the specific situation.

Frequently Asked Questions

  • Per Ontario Regulation (O. Reg.) Sections 51-53 and 420/21 of the Occupational Health & Safety Act, along with specific Occupational Health & Safety Policies at McMaster Innovation Park (MIP), all health and safety-related incidents must be reported using the designated form.

  • If a critical incident or injury occurs and the situation is stable, you are required to complete and submit the incident report form as soon as possible.

  • By submitting the incident report form, you affirm that all information provided is true and accurate to the best of your knowledge. The form will be forwarded to the Health & Safety Committee at MIP for record-keeping and to determine the next steps, if necessary.

  • The Health & Safety Committee at McMaster Innovation Park reviews all submitted incident report forms to assess the situation, record the incident, and decide on any required follow-up actions.

  • You should report any incidents that impact health and safety at MIP, including but not limited to injuries from slipping on ice in parking lots, loss of consciousness due to exposure to hot weather, injuries from tripping on floor mats in common areas, and experiencing harassment from employees, community members, or visitors.
